Support Coordination

What is Support Coordination?
Support coordination is done in collaboration with NDIS participants, or on their behalf. It involves determining, organising, and gaining access to supports and services that are tailored to your individual needs, wants and preferences.
Our Support Coordinatiors can help you to:
- Explain what is and isn’t covered in an NDIS plan
- Plan the best way to use NDIS funding
- Connect participants with different community services and supports
- Prepare NDIS reviews
- Prepare for the unexpected
- Coach you towards independence
- Establish and maintain your supports
Choice and Control
Capacity building is an important part of Support Coordination. Your Support Coordinator will work with you to increase your confidence in different skills to help you work towards your goals.
